GLP-1 Weight Loss Medications and Muscle Loss: What Many People Don’t Realize

Research suggests that a significant portion of weight lost during GLP-1 treatment may come from lean body mass, including muscle tissue.^1,3 In the STEP 1 clinical trial published in the New England Journal of Medicine, participants taking semaglutide lost an average of 14.9% of their body weight, and follow-up body composition analyses showed that approximately 39% to 40% of the total weight lost came from lean body mass.^1 Similar findings have also been observed with tirzepatide (Mounjaro®), where clinical trial data suggested that roughly 25% to 30% of weight loss may come from lean mass.^3
This matters because muscle plays a critical role in:

mobility
posture

energy expenditure
and long-term weight maintenance.
Losing too much muscle can contribute to:
weakness
reduced metabolic rate
fatigue
poorer body composition
and a “smaller but softer” appearance.
For some individuals, rapid weight loss may also contribute to increased loose skin and reduced skin firmness.^6
Why Muscle Loss Happens During Rapid Weight Loss
The body does not exclusively burn fat during weight loss.
When calorie intake drops significantly — especially without adequate resistance training or protein intake — the body may also break down muscle tissue.^4,5

physical activity decreases
protein intake is inadequate
or resistance training is inconsistent.
